It works because some releases clip their audio at half way then mux the files together during encoding. Sometimes usually during the last couple frames the audio does not end when the video does. When you demux and remux it merges the audio first causing the audio to start a few frames ahead when the second video is merged, therefore throwing it out of sync.
By clipping the last couple frames you ensure that the video and audio end at the same moment. How many frames depends on how far the sync gets thrown when the second CD begins.
Clipping the first couple of frames from the second CD is sort of a precautionary, but is usually unneeded. I'll usually do this if the 2nd CD is still part of a scene from the 1st CD. Effectively removing the entire scene so no one notices the jump. Obviously the scene has to be short and unneccesary to make this jumpless merge effective.
